Transition Planning

 

Transition Planning in Unique Learning System® is designed as a data collection center for Middle School, High School, and Transition students.

The tools in this section provide information that will assist the student and service providers in progressing from academic learning in the early grades into the application of skills and personal preferences for transition years. The Transition Planning tools are designed to be completed by the student.

The student selects a response to indicate:

• Current Status (This is me now)
• Preferences (This is what I like)
• Abilities and Challenges (This is what I can do)

Five sections of planning assessment are included:

• Vocational/Employability
• Daily Living
• Community
• Personal Life
• Lifelong Learning​​

A composite report is developed that enables the student to develop a personal future plan. Each section may be completed over time as instruction is provided to assist the student in understanding the assessment questions.
